Sure there is freedom of speech, although it is curtailed in various ways. For instance, shouting "Fire!" in a crowded theatre when there is no fire is a criminal act due to the panic such a word causes - panics often lead to stampedes, which have crushed a fair number of people. It is also considered criminal to incite a riot - this again is from a public safety point of view, as riots tend to target anybody available. It is also criminal to disseminate hate literature, again for public safety reasons. It is also criminal to reveal state secrets, and in many places it is even criminal to reveal corporate secrets.
But aside from these special cases, speech in the West if fairly free. As for thought control, that is what censorhip and propaganda is all about. The Church is an old hand at this.

Freedom of speech means being free to comment on anything under the sun without fear or favor.
It depends on a number of factors: country, history, tradition, linguistic speech genres, conscience, and even sexual orientation. Hence, fundamentally, freedom of speech is non-existent!
You are right, Freedom of thought is ideally conscience and this is probably the only freedom no one can take away unless they brand you medically insane first.
