The test is very subjective and is entirely on the shoulders of the tester. Manipulating how the tester perceives you is a key factor. This could be done before you are even attached to the machine. As for the test itself, during the calibration stage, you should increase your stress output signals by puting yourself into a panicked state when you resond honestly and try to calm down during the times you lie. This will create unusable data. The machine will detect changes in heartrate etc, so if you believe you are lying, you will exhibit signs of stress. Obviously, there is a great deal more to say on this subject, which is why there are a wealth of books on how to deceive these tests.

Well a lie detector simply monitors your physical response to the questions, heart rate, temperature, blood pressure, sweat, etc. If you have control over yourself and can remain calm physically while taking the test, then yes.
Also pathological liars usually will pass a lie detector test.
Both of these are the reasons why a lie detector is not admissible in court.

Yes, although it is easier to invalidate the test. The most successful people against a lie detector test are psychopaths and sociopaths, people with no conscience, I believe. Lie detector tests are rarely valid in criminal proceedings because they are so prone to misinterpretation and artifacts. People may look guilty, not because they are guilty of the crime being investigated, but because they are hiding some other information they do not want others to know.

Just relax. The test measures physiological arousal, like increased heart rate and sweat. So, if you stay calm, you may be able to manipulate the test.
