"Operation Northwoods, or Northwoods, was a 1962 plan by the US Department of Defense to cause acts of terrorism and violence on US soil or against US interests, blamed on Cuba, in order to generate U.S. public support for military action against the Cuban government of Fidel Castro. As part of the U.S. government's Operation Mongoose anti-Castro initiative, the plan, which was not implemented, called for various false flag actions, including simulated or real state-sponsored acts of terrorism (such as hijacked planes) on U.S. and Cuban soil. The plan was proposed by senior U.S. Department of Defense leaders, including the Chairman of the Joint Chiefs of Staff Lyman Louis Lemnitzer.
So for all you people who keep insisting that the US government would never consider attacking its own citizens, do you still think that is the case?

In fairness, a lot of things have been planned but cooler heads have prevailed and the plans were not carried out. There were some experiements the CIA carried out against unknowing, unconsenting citizens of the United States, though. They dosed a few people with LSD and some of them, unaware that anything had been done to them, became convinced they were going insane and panicked and committed suicide. The government also tested germ warfare against unconsenting citizens of the United States, they thought the microbes were fairly harmless but there was at least one death, in San Francisco, I believe. Both these experiments against the citizens were carried out in the 1950's. Wouldn't it be nice if public SERVANTS took their oath to uphold and defend the Constitution seriously!

Bottom line: Presumably planned but not carried out. The "secretly planned attacks" didn't stay secret very long. Isn't America great? Freedom of Information Act, a 24/7 microscope of media and independent news people aimed at Capital Hill - and - admit it - nothing stays "secret" very long - not even republican burglaries or democratic indiscretions, let alone serious policies.

The U.S. government hasn't planned, but claimed at least one accident as terrorist attacks ("Remember the Maine" was the catch phrase of an accident claimed to be a terrorist attack that helped lead to the Spanish American War).
