It depends on where you will be building these websites. If you rent a building, then you will most likely need a permit to do business in that city/town. If your entire business is online , including all payments accepted from your customers and you are building website from your home, then you most likely will not need anything. You can double check with your city hall, since some area's do have different rules concerning such.

You will however, need to either establish an Corporation, LLC, or declare yourself a Sole-Proprietor. Make sure you keep track as to where your sales come from because purchases in your state must pay sales tax, where out of state customers won't.

I wouldn't think so. You might in some cities but I can't imagine why you would. If you're going to have an actual "business" with stationary, business cards and other forms of advertisement then you'll have to fill out a Doing Business As (DBA) form at your local court house or County Clerk's office. Usually there's a small fee, $15 - $50 or so, to file it. Basically all it does is say that you, John Doe, are going to be doing business as "XYZ Web Creations." This is so that, should someone decide to sue your company, they know who to name in the lawsuit, i.e. you.

A DBA will also allow you to open a business account at the bank (so that you can accept checks made out to the company name, rather than your own name), legally put the company name on stationary and business cards and anything else like that.

Depending on your local laws you may need some other form of general business license or they may even have a specific one for web designers. When filing the DBA the clerk should be able to check on that for you, or direct you to someone who can.
