As written the expression is ambiguous, however, our society has created a rule which says in such a situation you must multiply before you add. Following this rule the expression is equal to 3. I don't know what you mean by the "need to use four operations for this question." Is that something your teacher is imposing? Arithmetic is said to have four operations addition, multiplication, subtraction, and division. We can in fact get along with only two since to perform subtraction we can add the negative of a given number, and to perform divisiion we can multiply by the inverse of a given number.
Examples 3 + -4 = -1 does not subtract 4 it adds -4
6 x 1/3 = 2 does not divide by 3 it multiplies by 1/3
You will find somewhere in your studies that an algebra is a set in which its members have two operations defined on them. Just two not four!

You do times and divide before plus and minus.

1 + 1 x 2 =

1 + 3 =

4

Soon as you see a times or divide sign, you do that first, then you work on the rest of the sum.

Getting a bit more advanced, things in brackets are done first, then 'powers of', then times and divide, then plus and minus.

Thats how I remember it anyway, been a little while since I've gone through that stuff.
