Inform utility companies on both sides (don't be left without water when you move to your new place), put all your important documents in one safe place (birth certificates, marriage certificate, etc.), put sentimental/valuable clothing in a separate area. Pack flammable things in a big storage bin with a lid, remove the petrol and oil from your lawn mower. Blow out your sprinklers and turn off your swamp cooler if you have one. Drain it too. Store all your "under the sink" stuff in a plastic bin with a lid.
Change your address online with the post office (assuming you're in the U.S.) at usps.com for $1. Update your credit/debit cards, online accounts (like amazon.com, ebay.com). Contact companies you have credit with. Forward your new land line phone number with the new number. Make sure you will still have cell phone service with your provider where you are moving to. Contact people you have magazine/newspaper subscriptions with.
Clean out your fridge, turn it off and leave it propped open so it doesn't stink (unless you're taking it with you). Buy a huge cooler for your frozen and fridge food. Clean out under your fridge and stove, you may find something you thought you'd lost a long time ago.
Throw out clothing you haven't worn in 2 years, or sell it on eBay. Take care of any pet needs you may have before moving. Contact local schools for your kids, if you're moving really far.
Ask around locally for recommendations for dentists, hair salons, doctors etc. Get your medical records (or have them transferred). I wish I have just went and got mine though.
Back up your computer hard drive. Remove the ink from your printer. Contact cable/satellite t.v. company (for both sides). Make sure you contact your ISP if you're moving far away, ours got messed up (even though we talked to them).
Make sure you have your bedding where you can find it.
If you're not moving too far, throw a house-warming party 2-3 weeks after you move in.
And finally, don't get too stressed!
