Depends on what type of autism they have. Some people with autism don't talk and throw temper tantrums. There are varying degrees of severity in autism, and not every autistic is the same. However, not being able to tell when they are in physical pain, not having good social skills, not making eye-contact, and oversensitivity to noises are sometimes problems for autistic individuals. Change is a hard thing for autistics.

Mostly, prejudice.
Like all other people, people with autism are unique in the challenges they face, and have unique adaptations to those challenges. Common difficulties in the autistic population are cognitive as it is a cognitive disorder. Having autism doesn't make you immune to other afflictions either, so multiple disabilities are not uncommon ranging from further cognitive disabilities to physical disabilities. Due to autism's history as a "psychiatric" rather than neurologic disorder most of its effects are characterized as mental illnesses. I would say that the challenges are much the same.

Difficulties with communication & the way the person relates to others
Inability to make friends
Limited capacity to understand other's feelings
Confused - need clear boundaries
Repetitive behaviours.
However, all children are individual and may have some or none of these challenges, please be aware that all children are wonderful and no child can ever be 'perfect' in every way. Celebrate the child's strenghts and work on developing any weaknesses or areas for improvement.

Many have speech delays or communication issues in regards to the give and take of speech. My 4 yr old son is still struggling with this. Also there are social aspects of trying to fit in and not picking up on the social cues most of us take for granted such as eye contact and body language.

As others have said - communication and social cueing.
Another big issue is sensory input. Many individuals with ASD are either hyper- or hyposensitive to their surroundings. They have to learn how to adjust to compensate for their sensory integration.
This might mean if they have hypersensitive hearing they wear ear plugs or noise-canceling headphones. Or if they are hyposensitive and need sensory input, they have a "fidget" item.
Sensory impairments can be auditory, visual, kinesthetic, vestibular, tactile, proprioceptive, tasting, etc. Some people with ASD are hypersensitive to some sensory input and hyposensitive to others. My DD, for instance, is typically hyposensitive and seeks input (usually self-stimulatory). However, she is hypersensitive to certain smells (craft store glue & eucalyptus, garden store fertilizer and dirt, etc) and certain pitches. As she gets older, she is learning how to deal with the issues in appropriate ways.
So for many with ASD, it's learning what their sensory triggers are and how to adapt to the world at large.

Depends on how bad it is. Some are very functional. In general I would say communication and socializing.
