Whenever I get to about 4000 rpms (seems to be mostly in 1st, 2nd, and 3rd gears), the car surges and the rpms rapidly surge and die out, making the car lunge a hell of alot. I've got about 150k miles on the engine). Anyone know what this problem is?

A couple of things disconnect the vacuum line to the EGR valve and drive it again. Or oil in the distributor? Low fuel pressure. I charge only one hour to check out a car for a first time customer. See a Nissan Dealer who knows your car and have a look at the new Altima Coupe

could be a dirty fuel filter, or the fuel injectors, just from the description, it sounds like a fule problem some whee, so i would check all those componets first, if thatis not it then put it on a diogistics machine, and see whats up, good luck hope it works out for ya!
