Present the best evidence and strong allegations to prove what is supposed to be the cause of action so that a favorable decision will be granted.

The best thing to know about petitions is that no-one gives a toss about petitions.

If you must start one, first name, last name and address is the minimum, I would think, to give some confidence that the signer is a real person who has enough commitment to the issue to make his or her opinion public, but you would be much better to spend your time to write a well-reasoned letter from yourself, which is worth more than any number of random names.
