please answer do all people have to sign an agreement to certain terms (keeping the yard cut and free of debris) and if so and they dont do what actions can be taken.

All homeowner associations should have cc&r's that are the rule of the association. The above abrevation stands for Conditions,Convenents and restrictions. They should be drawn up by an attorney who deals in real estate law. They state what color the house,trim,doors,shutters may be,how lawns should be, also landcaping do's and don'ts. It also should cover street sweeping,yard debris, snow removal, etc.
Usually when the rules are broken, one of the neigbors calls the association or the board when it walks around to take notes files a complaint. The homeowner is then sent a letter stating what the violation is, to remedy the situation or the outcome will be a fine.
Also you decide on a monthly meeting with just the commitee or if the homeowners are allowed to attend. Be prepared to hold an open discussion for the homeowners to state their concerns. Other issues are private among the committee member themselves (such as costs, etc)
All homeowners pay a monthly fee for common area upkeep.

Please contact a real estate attorney (he can draw up the cc&r's) Have a general meeting with all homeowners, vote on a President, Vice-President,Secretary and Treasurer.

You need to hire an attorney to write the association terms. you need to nominate a president and treasurer.
This all needs to be recorded at the county courthouse to make it official.

Assuming that this is agreed upon by all the homeowners. you also need to agree on a yearly fee.

I don't know how many homes are included.

I never had to sign anything and my neighborhood has a Homeowners Association....

Get the neighbors together, for a meeting and discuss it.
