I am not sure that anyone has actually answered your specific question which I think stems from the following:

Well, there are many benefits to this sort of mindset and reputation. Being an "insider" in almost any sense is viewed as a bad thing. It has a bad connotation and many people try to avoid it.
When some politicians run part of their platform is that they are not a Washington "insider." So when media personalities attempt to say they are not in the mainstream media, it is more of an attempt to sell an image than an actual factual statement. After all, you are right, Fox is widely watched.
So how can it be outside the mainstream? It is not. But, people seem to trust a source more when they believe that source is not beholden to whatever their view of the rest of the "media" happens to be.
Frankly it is an overused line. If you have a telelvision show, whether it's on cable or a network, I consider you to be a part of the mainstream media.

No one considers Fox News Network outside of the mainstream media. Their viewership insures a seat at the table.
People may not agree with some of their tactics and personalities, but the numbers don't lie.

Mainstream means more traditional media. Fox's competitors are more center-left in giving the news, and much more selective in who they interview and who they don't and they deemphasize the conservative point of view.
Fox on the other hand likes to analyze the news in more of a debate style which is much more entertaining and fascinating to watch. Because both sides are free to express themselves in a debate format, views expressed on Fox are usually more representative to the viewers than on the traditional networks including CNN, CNBC, and MSNBC.

FOX NEWS is more balanced than any other tv news source. They do try to present both sides which is a concept CNN AND MSNBC hve never dabbled in.

Because they are only on cable and offer a balanced news show. The others are "mainstream" in that they are on regular channels and don't offer fair and balanced news for the most part.

2. FOX """NEWS""" is not Journalism, it is "Entertainment Programming" like Wrestling. At least according to fox networks lawyers.

Yes they are! They are diverse; reasonably accurate in their reporting and do cover the world. They also handle controversial subjects extremely well, and have some top reporters to boot.
