you need to go to the social security office ...and make sure you bring a birth certificate, and drivers license or miiliary id....but if yiou have lost it you should contact a credit bureau so they can put an alert out on it ..anytime anyone does a credit check you will be notified..i had to do the very same thing with my sons ssn..he lost it so its something i had to do..there is a fee when you do that but you only have to report it to one agency and all 3 credit bureaus will be notified you have an alert on your number the fee is worth it ..would rather pay that then thousands of un-authorized charges someone made by using your identity...the ss office will only issue a new card but will not do much or anything at all as far as protecting you from theft..trust me i have been through it..
and by the way you cannot do it over the phone...if that was an option think of all the identity theft..it would become worse than it already is..i believe you can download the forms though...actually you dont need any just the id and they do everything by computer..was just there 2 months ago and i didnt fill out anything jus gave them the id..

Visit your local Social Security office, to apply for the lost or misplaced card, usually opens on normal business hours, look in the "White Pages" directory, for more information. Or log to www.ssa.gov

www.ssa.gov you can get forms to download and you should have a social security office not too far(depending on where you live) im in the city so i know of 2 offices i can go to within 25 minute drive
