Not necessarily offensive, but perhaps confrontational. If a speaker used terminology you didn't understand, then you might ask: "What do you mean by this?" although you would probably add the word "term" or "expression" at the end of the question. If a speaker was going on a sexist or racist rant, then you might properly ask: "What do you mean by this?" or "Where is this coming from?"
