A coworker's father died in January and is responsible for his remaining debt. Before creditors are willing to work with her, she needs a death certificate.
She contacted the funeral home (Tennessee) in February who stated that they had failed to file the paperwork. She continued to contact them throughout the months and they give responses like "We will call you once the certificate is filed" without taking a number or getting back to her and "We are trying".
It has been several months since his passing. Who should she contact?

I think that she is looking for someone to report them to that will make it harder for this company to put someone else through the same situation as well as someone to contact as far as getting a certificate.

Here's who to contact:

Tennessee Vital Records
Central Services Building
1st Floor
421 5th Ave. North
Nashville TN 37243

Phone: 615-741-1763
FAX: 615-741-9860

If my memory serves here. A death certificate is supposed to be filed as soon as possible, but as least within 30 days of death.

An incomplete certificate can be filed (sometimes info is just not available). It supposed to be as soon as possible but no later than 30 days, with a follow up in six months.

Was there something special about his death? Was it murder? Did it happen under unusal circumstances? If so they may legally be prohibited from filing it so far. If they just haven't done so, contact either the States Department of Vital Statistics or the local health department. Depening on the state these are either done at the state or locally at the county/city level. Let them know you wondering why it hasn't been done and they'll check and see if they can answer. If it's one of the things I mentioned above though they might not be able to tell you.

State Attorney's office for that county or parish

also - you may look for the Tennessee Bureau of Vital Statistics - and perhaps get the DC through them

best of luck
