personally i don't trust reg cleaners, for 4 reasons:
1. you don't always know what changes are being made
2. you don't know what the impact of the changes will be
3. you may not be able to undo the changes if they break something
4. registry cleaning is a generic solution, and may not address your specific issues
if you have a specific issue that you feel would be resolved by a reg cleaner, consider the option of hiring an experienced computer professional (not your neighbor's kid who "really knows computers") to troubleshoot the issue.
the ultimate registry cleaning method is to reinstall your OS. this is a safe procedure when performed by an experienced professional (after all of your data has been secured via backup to external media, of course).
other (less drastic) solutions are probably available, depending on the specifics of the issues you are trying to address.
after having said all of the above, if you still want to use a registry cleaner, at least make a backup beforehand:
backup your system state (WinXP):
1. start | run | open: %SystemRoot%\System32\restore\rstrui.exe | click OK (or start | all programs | accessories | system tools | system restore)
2. select "create a restore point" | click Next
3. restore point description: enter some descriptive text here (e.g. "Pre-Reg Cleaner Restore Point -- 24 May 2007")
4. click create

You can buy the book that tells you. Or if you have plenty of time you can do like I did when I was down hearted and so alone I picked out one file and removed it. Shut down and restarted the computer and if it didn't then I reinstalled Windows 98 from the CD and started over. Windows 98 se came out and I found Registry Mechanic and tried it. Did not like it I went back to removing the one's that the book said could be removed safely.
One registry function at a time...
When windows 98 was replaced I got Registry Mechanic, with Windows XP and, and,,, later...
I purchased Max Registry Cleaner and dumped Registry Mechanic.

I use CCleaner. It has never given me any problem. You can clean the registry using its 'ISSUES' menu. Before it cleans it will ask you whether to backup or not. If you are afraid of malfunction, then do the backups. When I first started using CCleaner I did all the backups by saving in 'My Documents'. After about 2 months of using it without any malfunctions I deleted all the backups. Now I don't even do backups anymore because I have been using it for some 6 months now without any problem. It doesn't install any spyware, I have check with Windows Defender as well as AVG Anti Spyware and it doesn't run at start up nor do realtime scanning. its true, there are many registry cleaners out there but none could be so reliable, as we all know, registry is the heart of our system, once it is damage, it can cause unusual errors which sometimes lead to software reinstall or the worst an operating system reinstallation. so before taking a step, always, and be always be careful. "Always Backup your files"
Manual way, if you know a value you want to remove you can do it your way, by doing this, your getting a little closer to you system. do this by clicking start, then run and type regedt32 then enter, you will then be redirected to the registry editor. here you can back it up and have your cleanup but this work is very tedious and dangerous,
Programs - there are a lot of software out there that tells you they can clean up your registry, remember if you know the values you want to delete then go on, they usually ask you what to delete first, unless you set is automatically, they do have their backup but i suggest you have your own too.

http://www.winaso.com/ I use the free version at least 3 or 4 times aweek. It fixes 10 items at a time and the free version never expires.You can buy the full version that does the same thing if you want to fix everything at once. It really speeds things up and has a registry defrag program in it also.

I've try quite a few cleaner programs, but the only one I use now is http://www.iobit.com/AdvancedWindowsCarePersonal/index.html
It's a free utility, does a drive cleanup, registry cleanup, and also immunizes against a number of spyware issues.
