One is the legal mother, maybe the other must adopt. I don't believe they will allow 2 mothers to be listed on the birth cert.
Good question, I feel you will probably struggle to get this accomplished.
I wish you luck.

well i know in the state of ct that orginally there is only the birth mothers name but you can do something sorta like second adoption and than the other mothers name can be on the certificate as well. im sure it all depends on where you are living. good luck

Even though I am guessing through my teeth:
I believe in California USA and in some other countries, you can have both on an adoption certificate.
In the case of your own child, I think the hospitals will just ask "What name shall we put down for the father?" Then you can just have them write the other mother's name in. If they give you any flak, say, "Yup, that's his name." If they say, let me see a birth certificate/driver's ID, ask if they ask that of all the mothers. (Note: I think you can put ANYBODY'S name down for the father. How else can unmarried girls put down a guy's name even if he isn't there?)

How can a lesbian have a baby?( I know, a sperm donor is required, or adoption) "Lesbian Parenting" are mutually exclusive words. Studies have shown since the morality of this country has declined to even allow this situation exist in the first place, that the few children who have been studied show them growing up terribly confused. For lesbians to "raise" a child is one of the highest forms of selfishness.
