It could have happened at any point or for a multitude of reasons however, i would leave him where he is but pay attention to the fish more often the first signs of any disease should be addressed immediately. Why i say leave him where he is is because most illnesses are bought on by stress so moving him while his defenses are breached is not bright.
After awhile the skin will deepen and provide protection till or if the scales grow back which sometimes takes a long time. however the fish's natural slime should deal with any problem.

Hmmmmm, well if your fish is a black lion head (lionheads are fish that have this odd massive brain on the top of the fishes head) it's ordinary for the black lionhead to seem like it's losing scales cause black lionheads change color! They usually change to a yellow-orangish color! Good luck! If your fish is not a black lionhead, well then go to a vet and ask for some help.

He could have knocked it against a sharp object ,Maybe an ornament/deco sharp plant maybe? Or i could be a fish having an argument,Or fighting or somthing, It was probibly just knocked off an ornament or plant tho, Im not to sure if they will grow back, You cant fix it, But id remove it to a hospital tank to let it heal properly

You need to watch them closely for a few days and see if they are scraping on objects in the tank even the bottom. If they are they could possibly have a parasite or even fungus. My suggestions are to watch them real close for a few days and if you observe any odd behavior that you can't explain. Get on YAHOO ANSWERS again. Hope this helps.
