My left leg from the toes, to just below the knee swells so bad
that it makes the muscle hurt. Also, the toes and ankle on the
right foot swell at the same time. Is this excess water or a vein
blockage. Does the pacemaker I got in 03 have any thing to do
with this problem?

Tell your cardiologist! It may have something to do with the underlying condition you needed the pacemaker for.
You may need a diuretic to help reduce the amount of fluid you retain and also your heart may not be pumping strongly enough to circulate the blood so it is pooling in your lower body.
Keep your legs raised whenever possible, avoid salt and get to your doctor as soon as possible.
