There is no algorithm. Area codes and prefixes (NPA NXX) have been assigned for many years in 10,000 number blocks as per the North American Numbering Plan.
Recently they have moved to 1,000 number blocks instead, and it makes things more difficult.
To find out who the number was INITIALLY assigned to, there are a number of services that update this information, and you can see the phone company that owns it.
Unfortunately, to complicate matters, local number portability allows you to move your phone number from one company to another. What used to be my home phone number is now a cell phone number.

No. Mobile providers periodically were assigned numbers or even buy up phone number blocks from landline providers.

no but you can try looking up the number on anywho.com.. its not available to cell phones yet but if the number comes up on there then its a land line
there is anywho.com and yellowpages.com
