An ordinary physician - family doctor-type - does rectal exams to palpate the prostate gland for lumps and enlargement. It's not a "fun" activity, but is a very good test to have on the same basis as a general physical exam, and especially if you are having difficulty urinating.

Any doctor can do it but usually you start at the bottom of the food chain with a family practice doctor who if need be will refer you to an appropriate specialist. That is also the least expensive way to do it and often insurance will demand that you follow that path also.
