I've been asked to illustrate a front cover and then 12 inside pages. The cover is the main artwork, then the pages are like one main icon plus some bits from the cover. The whole thing needs to be designed as well as illustrated so I need to put backgrounds and fonts etc into it.

Depends on how long it would take and your skill in the field and the person you're doing it for.

I'm a designer. If I was doing a professional job for someone that was to be mass-produced, I would charge about 700 to 1000 dollars if it was all original work. If it was for a friend of a friend, or something that was only going to have a few copies, like for a special event or a smaller group of people, then I would charge 300-500 dollars.

Of course, the types and quality of illustration must be taken into account - a cartoon-like or abstract illustration is worth less than a photorealistic pencil drawing... each of them takes less time to do. If you are a fast illustrator, then you might charge less than I would, since I'm better at design than I am at illustration.

Whatever price you reach as a lump payment ie. 13 x 500 or whatever - try and negotiate a percentage bonus on each copy produced/sold - i.e. 2% per calander. If you don't ask, you don't get.
