Well, she doesn't stick it right out, but kind of wedges it between her gnashers.
I thought she was snapped chewing gum with her mouth open in a chavvy way, but no i think it's her tongue. She's like this in most pix i've seen.

May be a habit. Sometimes it like a Trademark (Hard to explain). But, something a person will do that is related to them by people when it seen. So, everytime you or someone else does this people will think of Lilly.

I do that. I dont think its any other reason than she feels uncomfortable when knowing people are taking pictures of her so feels she has to do something like pull a face, stick out her tongue etc

Its a poor show. Very childish. Shes a dirty little coo that one and needs a gid wash, but this tongue thing is irritating. I reckon its a insecurity thing that will go away with time. Hope she follows it.
