What is it, and how can I get rid of it?

It could be one of several things, but the most likely two causes are bacterial infections. Furunculosis and columnaris can both attack this area of the fish and cause exactly that appearance. Both can be treated the same way. I would suggest treating with Furanace as the first choice and Maracyn TC as the second choice. Just treat as per the directions on the package and treat for 10 days to insure that the infection is cleared up.

Could be an ulcer, wound or a anchor worm. Check your water perameters immediately. I would raise the heat to 83 degrees and add 3 teaspoons of salt per gallon. That is after you change enough water to right the water perameters which should be ammonia and nitrite = zero, nitrate under 20 ppm. Ph stable, should always be monitored and maintained the same level. Anti-biotic food will also help. Be cautious of treating the water with chemicals, I've found they do much more harm than good in most cases and are usually not necessary, even though many people will tell you that's the solution. Remember this, it will take as long for the fish to heal as it did to get sick. So it will not go away over night. Keep the water as perfect as possible to avoid illness. Feed only the best food and replace it monthly as it does go bad. Flakes are usually never good food, they are made with mostly cheap filler and little nutritional value. Flakes go bad very quickly too.
If it is a anchor worm starting to protrude you will see a little white worm like thing come out of the middle of the spot. Simply hand catch the fish and hold firmly, get a clean set of tweezers, grasp the worm and pull it out. Dab with a cotton ball soaked in hydrogen peroxide. Be careful not to get the peroxide in the gills or eyes of the fish. It will not hurt the fish or water other wise. Good luck to you.
