It's sort of a heat of the moment thing where no one is really thinking about what they are doing other than sex. When you don't use a form of pretection or birth control, you are likely to get pregnant. I don't see why it is considered an accident though because you both know the consequences before the act is done.

Lots of ways. Sometimes they just mean "we got carried away and forgot to take precautions..."
Or - Condom breakage, forgot a pill, decided it was a "safe" time, he lied about the vasectomy, too drunk to use the condom or diaphragm properly, she said she was on the pill... pick an excuse.
Also, there's the occasional failure. If the condom had a slight leak, from the factory or mishandling, maybe you wouldn't notice. The pill is 99.5% effective, but that means that even used properly, that 99.5 means 1 out 200 couples who use it will get pregnant in a year of regular use.
Also, there are drugs, like some antibiotics, that stop the pill from being effective. And, I've heard of husbands who mess with their wife's birth control hoping to have a baby. (Wasn't that on an episode of Desperate Housewives last year?). 7 out of 28 pills are "do nothing" pills during the period. save and rearrange those, or find something similar... That's why foil blister packs are better.

Well, maybe if you didn't realize you were having sex somehow and thought that you were just doing some new yoga position with your partner.
If you are old enough to get pregnant, then you should know how it happens. I would think that broken condoms and birth control would not be an "accident" because you are aware of the possibility of this sort of malfunction and the obvious possible consequences.
Just my opinion, of course.

Condom broke. Thats the only accident. Missing your pill is not an accident, thats just dumb. Think about it people, if you really didn't want to have kids, you would have been super protected. BC pills and condoms, shots and condoms, IUD and condoms... Always a backup...And if all else fails, abortion. You live with it cause you were just dumb, not because you had an accident!

I would say that if one is practicing birth control correctly and consistently, and still becomes pregnant, then that qualifies as an "accidental" pregnancy.

there are no accidents, to have sex is a choice and when yu make choices you have to live with the fact that their are consequences sometimes.

if you know that having sex causes pregnancy then it's no accident...

about the condom break is joke come on people are so ignorant and need to get some sex education , you are peer teens and say condoms break so when they come to have sex they say " ok condom break so i don't need use of it" .
