You may be able to get a living will (healthcare directive) from your local hospital's website for free - so check there first. Here is a link to one that cites the California Probate Code, as an example: http://www.sharp.com/patientforms/index.cfm?id=93

If you want the form on your own computer so you can save it and edit it as necessary, WillMaker is the software you need. It is available at http://www.nolo.com - along with a number of self-help legal books. The software is not expensive, and you can create other documents, too, including wills, trusts, powers of attorney, etc.

It's just a really bad idea to use legal documents off the internet. If it doesn't work out, you won't be in any shape to fix it, and your family will be paying thousands to an attorney to fix it. It's safer to have an attorney do it right in the first place.

Legal documents aren't cars. They simply are not a generic product suitable for buying this way. They need to be tailored to your circumstances, with someone that will know if the state law changes the day before you execute it.
