What you have described usually happens when you have used your mouse for a long period of time. If you are unable to clean the surface of the mouse or the optical light, then it time to purchase a new mouse.

I've seen this occur with some optical mice. It happens when there is some pattern on the mouse pad or surface that confuses the optical sensor. I've also had a mouse with a flaky cable exhibit the same symptoms. It may be a malware/virus issue. Run a free scan at trendmicro.com or download, update and run AVG Free antivirus.

No one has suggested this yet: If you have a ball mouse, the internal wheels accumulate dirt and are easily cleaned by taking out the ball and just scratching off the funk.
