Well, the major cause of fatalities is drowning no matter how you get into the water. Large waves are dangerous for a small boat, so is loading a boat beyond it's weight rating., but the most dangerous is boating without a proper life preservers for every member in the boat.

Many will say wearing a life preserver takes away from the boating experience, but so does drowning. It is a good idea to wear one when on the water. Even with a non-fatal injury received in a boat-to-boat accident, it is the life preserver that can give you the chance to survive by keeping your head above water, even when you are unconscious.

If you or the people you boat with are those who refuse to wear a life preserver because you feel you can swim well, a good idea is to at least get the life preservers out of the boat hull storage, hand one that fits to everyone who can swim, and make them sit on it so it is there if they find themselves in the water. And anyone who cannot swim well, give them no choice, wear it or watch you from shore.

It is also a good idea to have a throw-able rescue flotation device handy in a boat to toss to anyone in trouble in the water, even people not a part of your crew.

Remember, in the water, best to count on only you for survival. Help in water takes time to arrive, and anyone in the water with you, are normally busy saving themselves.
Happy Boating :)

Curtis gets the answer right. Wear your PFD! Inflatables are comfy and small and under $100.
And don't drive drunk. Drinking is the primary reason for drownings where I live. I don't boat without a beer near but I keep it cool and wear my PFD and I'm still breathing air. I love breathing air. Hate trying to breathe the h2o

Gotta go w/ booze.
However....
Interesting fact from a coast guard article called "whizzing over the rail"
Many drownings are caused by taking a leak over the side .
CG said the proof is in the numbers of floaters found with their fly's open.

i bet the statistics will show teenagers being daredevilish are as to blame as alcohol. with the amount of fishing done these days, falling overboard and drowning is probably the most common.
