Take or mail the recorded marriage license to DMV and Social Security. Once you have changed your ID, banks, doctors, and the like, will go from that. Any thing else it should be as simple as telling them your new last name.
Changing your last name is up to you. You can use it or not. I changed my name for my first marriage, then had my maiden restored in the divorce. I am happily married now, but decided to keep my maiden name.

Why would you want to? Keep your own last name. When you reproduce, name your child a first name, your surname as its middle name, with its last name that of your husband. Taking a husbands name when married is becoming a custom of the past. The same last name does not guarantee that the union will function with greater commitment, than it will if you maintain your surname. However, it's your choice, and if you chose to make the change, start at your county courthouse

My last name automatically changed to my husbands when we were married. I just had to have it changed on my driver's license and such. I do know a doctor that when she got married opted to keep her maiden name, for business reasons. But, she had to fill out paperwork to send in to the state. You should check with a lawyer or wherever you got your marriage license.

When you get married the womans last name automatically changes to her husbands unless you have stipulated otherwise.
I would go to Social Security and get a new card--same number--with your new last name.

Check with a lawyer.
If you are legally married then under normal circumstances (I know - what is normal these days) you can automatically take your husbands last name (Mrs. xxxxxxx).
