Pre-op --- transgendered people who have no yet had sex reassignment surgery and are generally on hormones.
Post-op -- transgendered people who have had the sex change surgery and now possess the genatalia of the sex they have 'become'.
For men who become women, it's actually relatively easy..the shaft of the penis is basically inverted into where a vagina would normally be, and the head of the penis is converted into a clitoris. Hormone therapy is used to develope breasts and some MTFs get implants.
For women turning into men, it's a different story. A full hysterectomy is performed as well as a double masectomy to remove the uterus and breasts respectively. The clitoris is converted into the penis head, and skin from the body is used to create a penis with the help of male hormone therapy. It's a pretty complicated process and the penis is very rarely the shape/size of a natural one.

Pre-op, means that they have not had sex reassignment surgery, yet. They are still the same sex that they were born as. Post-op, means that they have had surgery and are now physically a new sex.
