I'm sure Surveyor meant to say that there were no Apollo program astronaut fatalities during a flight.
Gus Grissom, Ed White and Roger Chafee were killid in a spacecraft fire on the launch pad during a test for the first Apollo test flight.

Apollo 13 was the closest. It had an O2 tank explode, but the entire craft did not blow up. They made it back safely. Or at least alive. They were all sick from being so cold for so long, and were suffering from dehydration due to having to severely ration their water intake.

Only Apollo 13. A safety check fused a LOX storage tank heater to "on." The tank ruptured on the way to the moon. "Houston, we have a problem."

13 had an oxygen tank explode, but it still made it. Technically the united states has never lost anyone in space. Even discovery was considered on earth since it was in the atmosphere.
