It's got to be Anthony Hopkins as Hannibal Lecter in Silence of the Lambs. Anyone who gleefully eats people is top of the psychopathic heap.
His calm delivery and haunting eyes are unmatched.

has to be kathy bates as Annie wilkes ( she won an oscar for best actress because of this) in the movie Misery based on the book by stephen king. In the book Annie is such a great psychopath but kathy REALLY makes her look crazy in the movie exactly how i had imagined her to be. a Delusional, manic depressive, mood swinging torturer!

for me, it would be jim carrey as the riddler. i think he was in batman forever, i like it cuz you never see him as a villian and he's really good at it (insane, sinister, evil, genius)
