If a lead is larger than 3 runs when a pitcher enters, then the only way you can get a save is by pitching 3 or more innings of relief.
So, there are two ways to get a save:
1.Coming in and protecting a lead of 3 or less runs.
2. Pitching 3 or more innings of relief in a victory - usually seen in a blowout.

A relief pitcher is awarded a save when he meets all three of the following conditions:
He is the finishing pitcher in a game won by his club; and
He is not the winning pitcher; and
He qualifies under one of the following conditions:
He enters the game with a lead of no more than three runs and pitches for at least one inning; or
He enters the game, regardless of the count, with the potential tying run either on base, at bat, or on deck; or
He pitches effectively for at least three innings.
Under the last condition, the official scorer has some discretion as to whether or not to award a save. This is rule 10.20 of the Major League Rules.
No more than one save may be credited in each game.

The closer or reliever comes in to close the game. If he comes in when his team is winning by 3 runs or less, then he can attain a save. If his team is winning by 4 or more runs, then there is no save opportunity. Even if he gives up a run, there is still no chance for a save, unless a new pitcher comes in while the team has a 3 or below run lead

Technically, any time a relief pitcher enters the game and the save situation (types 1 or 2; 3 is innings length-based) is not in effect. In general, though, the lead is too big.
