Report him, and get a recorded statement if possible and if legal.
If I was king, I would toughen up the food industry to include prison terms for the slightest of violations, including serving regular coffee instead of decaf.
Any form of food tampering or unsanitary practice would mandate a 20 year sentence.

Report him. Then maybe invite him over to dinner, do the same thing, maybe rub a little butt into the pizza or a that last drop that never quite makes it into the loo into his bread sticks. When he's done eating...tell him what you've done! He's not much of a friend if he'd do that and he knows that people you know eat there and he could be making people deathly ill. What a gross bast@@d!

No,don't report him, he made a mistake, a big one, granted, but still a mistake. If anything, you should blame the people who were selfish enough to go to his restaurant and put him to any trouble in the first place. He probably knows it's wrong, so that's OK. (Are we thinking on the same wave length?)

I don't understand why you have to even ask us this question. This should have been reported right away...it is a HEALTH LAW and if the public found out it could have serious affects on the business...........REPORT! Could you get this person to repeat this and either have a witness over hear or tape record it?

yes, you should report him. but to keep him from knowing that you did it and then getting mad at you, you should say..."i don't know who would report you!", when he asks who turned him in. it is a health violation not to wash your hands. yuck!
