All the years of being told what to do, along with all the criticism we get from others? Causing them to constantly go on the defensive?
If they don't outright block out what we say. They only let us get a few words out, and then start interrupting, so we can never get to the point. And then they think we said what they think they think we said. Instead of what we were trying to say.

I'm unsure if that's the biggest reason for miscommunication.
One big reason, for instance, is language barriers, which occur even between native speakers of the same language because of different dialects and colloquialisms.

I would agree that people not listening is one of the biggest reasons for miscommunication. I know exactly what you mean.
I encounter this every day, when talking to strangers, or to friends, and family.

Many people do hear what they want to hear. They ask you a question, then don't listen to your answer, for instance. I mean literally don't listen, but talk over you. Leaving me wondering why they asked the question in the first place.
It can be incredibly frustrating at times. I have people in my family, who I indulge by simply listening to them; because I've learned by now that all they want to do is talk, express themselves, and just need to get it out.

Most frustrating is when you are trying to sort out something simple and practical, like a time to meet, or where to go, and you just get nowhere, because of someone who simply doesn't listen.

Because often the truth hurts and some people block it out because they think it will hurt less and wont do damage to their self ego.
When they think you have said something they only wanted hear its called selective hearing and times like those require a hammer and a chisel to drill the words in they actually needed to hear not the ones they ego made them believe

people only hear what they want to hear-they also have selective hearing to dim down the hard bits and big up the good points, its bad but we all do it!-its harder to except things just as they are, we always need to explain things or know why.
