Leatherface (Texas Chainsaw Massacre), Buffalo Bill (The Silence of the Lambs) and Norman Bates (Psycho) all had elements about their characters which were taken from the real life serial killer Ed Gein. Wolf Creek and Open Water are both speculations based on real cases. An American Haunting is based on The Bell Witch, which is an acual documented haunting from the 1800's (though how "true" it is remains to be seen). Also, the Blair Witch Project was loosely based on the Bell Witch, but I don't know if that would count. Gothic was based on Lord Byron's challenge to create a horror story which lead to Mary Shelly writing Frankenstein.

Supposedly The Exorcism of Emily Rose is based on a true story. That movie scared the s.hit out of me.

I heard (I think it was through the DVD commentary) that Nightmare on Elm Street was based on some kids in Japan who were dying in their sleep. They would use coffee to try and stay awake so they wouldn't die.
