there is no certain "size"

It depends on your body.
It should be in porportion to your body.
If you're fat/wide, your head should be, too.
It you're skinny/petite, your head should be somewhat small.

if you are 3 and have a head the size of a horse than it's considered big, if your 40 say, and have a head the size of a mouse then it's considered small

My head size is 23 " which is a 7 and 3/8 or 3/4 hat size, and an XL, and I am a girl [ and only 5'4"! ] I wore the biggest grad cap of all the girls I graduated with [ over 100 girls and over 100 guys ].
I have a bigger head than my husband or my two sons, all of whom are bigger than me.
My brothers' head is even bigger, but he is 6 foot and his shoulders fill the doorway.

When you get locked in a bell tower, I'd say either your head or hump has gotten too large. Look at poor, unfortunate Paris Hilton, her head got very large, and theyre going to lock her away.

I'm sure there's a medical determination for this, but for the sake of saving time, let's just say, for me, it's all relative to the whole body of the person. The head just needs to be proportionate.
