...why, you just described the basic plot of the 1981 Roger Corman produced, sci-fi/horror flick, known as "Galaxy of Terror", starring Edward Albert, Erin Moran (aka 'Joannie' from 'Happy Days'), Ray Walston (right before playing Mr. Hand in "Fast Times at Ridgemont High"), Sid Haig (of "Spider Baby", and recently, of "The Devil's Rejects"), and a little-known, up & coming character actor known as Robert England, who would soon put on a certain dream demon's razor fingered glove...
Replent with stock footage from several other Roger Corman films, the plot of the film is exactly as you described: the members of an elite space crew, investigating a distress signal from an unknown planet, are one by one, subjected to horrendous and monstrous apparitions, based on each of their fears; their ability to survive is soley based upon their ability to conquer those fears....which obviously they are unable to do so, and are dispatched most horrifically and grisly (the rape of one of the women, by a giant worm, is quite...eh, interesting, and Joannie Cunningham's death by crushing is quite ugly, to say the least!!!).
Unfortunately, this film has yet to see release on DVD, and the VHS tape has been out of print for years (mine is just about worn)
