The only options to answer are: Single, Married, Divorced, Widowed, and Separated. But what if I'm in a relationship, neither married nor single? Should I mark the box for Single anyways?

Legally if you are none of the other options, and there is not a box for in a relationship you should mark single.

It's marital status, it not relationship status. Do this by elimination..if your last spouse hasn't died, you aren't widowed. If you're not married, then you aren't married. This leaves you two options...if you've never been married, you're definitely single.

However, personal rule of thumb, once you've been divorced as long as you were married, you can either put down divorced or single.

if you're in a relationship but not married, that means you're single for the purposes of that question.

Yes because technically until you say "I do" and there is a wedding ring on your finger, you are single.
