Was what you were making a claim on covered by Medicare? Did you need prior approval to get this claim approved? Did you fill out the paperwork wrong?

We'd need to know what the service was, and the reason they gave for denying. It could be that it required pre-authorization and you didn't do that, or it was a service that isn't covered, or that service was rendered out of the US (medicare/aid doesn't cover any services outside the US), or numerous other reasons.
