I believe Octavio Dotel did it a few years back in an interleague game against the Yankees. If I remember correctly, it was part of a no-hitter by about six Astros pitchers after the starter left due to injury.

Yes. If a pitcher throws a dropped third stike and the batter reaches first safely, I suppose he could stike out more than 3 batters in an inning.
