use them in restaurants they won't charge you. you should be able to use them in shops like cash as well.
not to sure if the hotels will charge you.
I always have a good mixture of cash and the amex travellers cheque card, that thing is great.

Ask American Express. They will have a list of approved agents

Difficult, they are all there to make money. Look around for the cheapest rates.
