The correct answer is "rang" but only b could be correct, if there was more context.

"Why were you awake at that time of night?"

"The phone had rung and startled me."

None of them, the correct form is "rang" unless you are talking about an action in the past which was interrupted by another past action in which case you would use the past perfect tense. For example: "The phone had rung and startled me just as I was falling asleep."

No choice is correct if you want grammatical agreement with "startled." But the the least wrong answer is "had rung." Otherwise, "rang" would be it.
