The most difficult thing in life to know is who you can or can't trust. That one gets me every time. You put your trust in a person, and they turn around and kick you in the teeth. On the other hand it has happened that I couldn't imagine a person being trustworthy, and they come through for you like no other. When to walk away is a good one too, like the first answer, but I have to say it is most difficult to know who you can trust.

Knowing that I was talking to a man who knew he was soon going to die from bone cancer, knowing what to say.

Knowing we are mortal, living with the knowledge that one day I will be old, and I will die, knowing my daughter will die, realising a family pet only has a short lifespan, no matter how sweet they are.
