Yes, it makes the potential employer much more likely to remember you from all the other people interviewed if there is a photo included.

If you apply for a job through email then a photo is almost a must since it gives the employers a chance to sort out people with a style that's incompatible with the job. (People with multiple facial piercings for work at a meat counter or something for example.)

No, it is highly inappropriate. And illegal for a prospective employer to ask for one, unless you are trying out for a job on TV or theater or a modeling job.
