The process for getting a PhD in Canada, the US or the UK is pretty much the same. You can get information about applying to a school from its web site. Usually, you need to fill out an application, write a one or two page cover letter explaining why you want a PhD at that particular school, provide your college transcripts, take the GRE Exam (and possibly an advanced subject matter exam) and get two or three letters of recommendation.

Most of the better programs provide funding for their PhD students. It is often in the form of teaching assistantships, research assistantships or fellowships. For my PhD at Berkeley, I had a fellowship during my first year, and alternated semesters as a TA or RA. This funding usually pays for tuition and fees and provides enough to pay for room and board.
