well...one suggestion, don't think about it too much. if you over analyze the situation you'll ruin the first kiss. im pretty sure you've seen the movie "hitch". will smith mentions that the first kiss tells a girl everything, well... it does. it tells us if theirs chemistry and if your a good kisser LOL!
but seriously...the yearning the tension is going to make the kiss very special if you let it happen on its own...k.

Well, I suppose that would depend on your age. Some girls chose not to kiss on the first date. However if she agreed to our with you again I would have expected a kiss at the end of the second date. Or she may wonder whats wrong with her. however if the two of you are say fairly new to dating than she may be hesitating out of fear. I still say you should be able to kiss her by now if your truly interested and vice versa.

well if you haven't kissed her yet your a little slow... no offense... but your supposed to kiss her the end of the first date to show you had a good time with her... best to kiss her the next time you two go on a date so she doesn't get the wrong impression... But remember kiss her gently on the cheek or lips... you don't want to be to violent or she might get the wrong impression there too.

honsetly it depends on how she reacacts to you. if shes like all over you go for it, if she's shy by the end of the 3rd date should be alright if shes into you she wont have an issue kissing you, and if shes not into you then she should stop goin on dates with you

That's all up to her, don't rush it. Just be thankful that you have found yourself a "good" girl whose parents raised her right(which you won't find amongst these harlots nowadays). I'm sure it will be worth the wait if you really like her.

personally, i think that you should get to spend more time with her, get to know her a little better, more than just 2 dates. even if you are comfortable around her, she might wanna wait until she is sure about you.
