I am trying to have a ceramic fairy made with maybe the face or clothing of a relative I have in a picture.

You should first try to see if there are local artisans in your area that do this kind of work. Maybe try a craft fair or two, or post a notice on your local craft stores bulletin boards.

You may also want to try http://www.etsy.com/ They have a ton of artisans that deal with every kind of handicraft and many do custom work. It would be worth contacting a few to see if they could do what you're looking for.

In addition to Silver_Star's answer above, I would also recommend going to your local community colleges' art department and talk with the art department's teachers and advisors to find if they have a sculpting student willing to take on the job. Of course, wer'e not looking at something that a professional is making, but your budget may appreciate these efforts. And it would add some variety to that budding sculptor's portfolio.

google for polymer clay artists, There are a lot more of them, and they will be able to create a fairy without having to build a mold first.
