Well, most women do not die in childbirth.
If they do, it's usually because of hemorrhaging of the uterus or through an undiagnosed complication (like toxemia).
In under-developed countries, the cause is usually lack of prenatal care and then complications (breech birth, placental problems, etc).

There is little that can be done. Just hope for the best and work with technology when possible.

There are alot of reasons for a woman to die in childbirth. Bleeding is most common, infection, complications due to prior existing medical conditions, etc.

Life and death ar the great circle, it is sad, yet unavoidable to some degree.

This used to be more of a problem a century ago when there were more home births and proper hand washing wasn't an issue with doctors and no gloves were used. It caused alot of uterine infection and then death.
These days it's really pretty safe to have babies. Women rarely die as a result of giving birth.
