The guy who said Aramis Ramirez did it is wrong. I couldn't wait until I went home to find my scoresheets of the game, but I eventually found what I wanted on ESPN.com and is linked below.
It was Game 5 of the 2003 NLCS and, yes Ramirez did hit a grand slam in the first inning, he was batting fifth and Mark Grudzielanek (hitting second) struck out swining. But Lofton, Sosa (how rare is that?), and Alou all walked only to be followed by Rami.
Anyway, to um answer the question ----- I have no idea.

i am not sure of the date but in game 1 of the 2003 N.L. championship against the Marlins Aramis Ramirez(4th batter) hit a grand slam after 3 walks scoreboard after 1st 4-0 with only 1 hit!
