Yes. The operation is very reliable, but not 100% perfect, and there are some failures. Sometimes the cut sections of the tube can heal. Sometimes there is just enough space for a sperm to make it through the tube to the egg, resulting in an ectopic pregnancy. Other times it isn't clear what went wrong, but somehow the woman becomes pregnant.
The article I linked to claims 1.85% failures over a ten year period.

Some doctors just tie your tubes off so the process is reversible if you change your mind in the future. Some doctors make it permanent and cauterize, or burn, the ends of the tubes so that even if they happen to untie and meet back up with the other half, they still can't grow back together. Chances of becoming pregnant if your tubes were cauterized are a lot slimmer than if they were just tied off. It's impossible to say without knowing your exact situation, so if you're having signs/symptoms of pregnancy, definitely get to your doctor as soon as possible.

I did, twice. Failure rates occur when the tubes were clipped, rather than tied and burned. The clips fall off and the tubes are open once again. The tube ends can be crushed, increasing your chances of ectopic pregnancy .

There is a lady here who lost her children in the Murrah Building bombings. She had surgery and was able to have another baby afterward. I suppose it depends on the person and how good a surgeon you have.

Only if it wasn't done correctly. If you had one and you think you are pregnant, you need to get to the dr. asap because they baby could be stranded in your tube.

Possible, but not probable.
The odds are against it, especially if it was done correctly!
